The quickest way to receive assistance is to attend a scheduled library pop-up, where you can choose from the items available that day. Use the Individual Assistance Request when you need additional guidance or cannot access a pop-up.

Before you begin: The request form does not reserve items or guarantee assistance. Some requests may require a scheduled warehouse pickup based on the type of assistance or inventory involved.

The form collects household, ZIP code, clothing and shoe size, timing, transportation and other practical information for review and program tracking. It is not a custom shopping or pre-pull service.
